Role Purpose:
To proactively manage the overall installation, maintenance, development and support of all of Trust data and voice infrastructure in accordance with Trust, industry and national standards, with particular reference to ITIL Service Management. To be the technical design authority within the organisation for all data and voice equipment, structured cabling systems and advise the Corporate I.T. manager on strategy in the rapidly converging areas of data and voice communications and service delivery.
Role Context:
Alongside the other two key areas within the Corporate I.T. Department this role delivers core data and voice communications services to the Trust supporting I.T. systems, business and clinical processes. Specifically, this role has responsibility for the technical design, operational continuity and future development of all Trust voice and data communications systems that support key applications such as email and clinical information systems. Awareness is required of local and national initiatives, standards and best practice as well as industry standards such as ISO 17799, ITIL and BS15000 etc.
Must keep abreast of the latest IT Technologies; online marketing and payment systems, must be flexible and able to pick up new skills quickly. Needs to be both operationally and strategically focused and capable of making decisions with a corporate wide impact.
Awareness is required of private health service online, local and national initiatives, standards and best practices as well as industry standards.

- Management and supervision of attached assistant and/or graduate psychologists.
- To provide a qualified specialist psychology service to clients of the Pain Management Team, across all service; providing specialist psychological assessment and therapy, including group therapy, at the same time as offering advice and consultation on clients’ psychological care to non-psychologist colleagues and to other, non-professional carers, working autonomously within professional guidelines and the overall framework of the team’s policies and procedures. To utilise research skills for audit, policy and service development and research within the area served by the team/service.
- The post holder holds budgetary or management responsibilities as appropriate in the day-to-day management of assistants and trainees Clinical and Applied Psychologists under the supervision of the Consultant Clinical Psychologist in the speciality.
- The post holder will in conjunction with members of the Pain Management Team make an assessment of those individuals who would benefit from psychological input and will under the supervision of the Consultant Clinical Psychologist provide relevant assessment, therapy, group work, and family interventions.
- Associates will contribute to teaching, training and supervision of other staff, within the service. Associates may also provide clinical supervision to the allied health professionals e.g. physiotherapists, occupational therapists etc. in the team. They will also contribute through the service structure to discussion about relevant business and service developments.

Job Title: Crisis Practitioner (Amhp/Nmp)

JOB SUMMARY
1. To deliver high quality care to individuals who have specific mental health problems i.e. have a severe and enduring mental health problem and multiple, complex needs.
2. To act as a key member of the Multi-Disciplinary Team ensuring the effective assessment, (Risk Assessment and Risk Management), planning monitoring and evaluation of care giver to service users with severe and enduring mental illness in acute crisis, ensuring the effective supervision of staff accountable to the post holder, whilst liaising with a wide range of multi-agency professionals.
3. To provide the “gatekeeping” function as detailed in the Service Operating Procedure.
4. To provide Bed Management as an integrated function of gatekeeping function.
5. To provide rapid and holistic assessment, care and sign posting of individuals within the above group but also for those presenting with other urgent risk issues.
6. The work of the Crisis Team will be managed as a whole caseload though the post holder has crisis and care coordination responsibilities for a individual service users within the team.
7. To be a “Shift Coordinator” to ensure safe organisation of daily activity.
8. To organise and deliver a rapid intensive intervention service in order to meet the health and social needs of those adults experiencing a serious mental health crisis.
9. To effective working relationships with and provide specialised advice and information on mental health issues to a variety of other services/agencies/organisations and carers both within and outside mental health services.
10. To participate in and contribute to local and National strategies to improve services.
11. To carry out clinical and caseload supervision and appraise and produce development plans with other grade staff members.
12. Where suitably qualified the post holder will be required to perform Approved Mental Health Professional (AMHP) and/or Non-Medical Prescriber role.

RESPONSIBLE FOR PATIENTS/CLIENTS IN THE FOLLOWING WAYS:
- As a member of the crisis response home treatment and acting in the capacity of gatekeeper to the inpatient services: –
- Receive and general referrals via multi-service access point,
- Undertake gate-keeping assessments, as secondary supporting assessments after referral once a primary assessment has been completed by another service.
- Decide whether alternatives to hospital admission can be arranged for these clients.
- Ensuring that clients are treated directly by self or signposted/admitted to all other services that can meet their assessed needs.
- Attend MDT AND CPA meetings to facilitate and support early discharges from inpatient wards
- Develop comprehensive planned assessment of clients newly referred or re-referred to the team, the completion of assessment documentation, and the formulation of a problem outline treatment plan and the feedback of all this information to the referring agency. Also involves feeding back information to the clinical team at allocation meeting (should the client be appropriate for secondary team input).
- Frequent assessment of risk that a client poses to themselves or others, communication of this risk to others involved in a client’s care including those outside mental health services as appropriate and the effective management of risk e.g. by requesting an urgent visit by G.P or Consultant Psychiatrist and/or input from an Approved Mental Health Professional, police or ambulance.
- Short term, autonomous management (as crisis coordinator in the absence of care coordinator) of a number of team clients.
- Responsible for participating in caseload supervision and negotiating with the manager / deputy manager when one’s caseload is overloaded.
- Short-term effective assessment, planning, implementation and evaluation of every day care and crisis care for clients and carers on one’s caseload by use of specialised knowledge obtained through theoretical work and experience, requiring judgements and concentration.
- Short-term delivery of specialised care, support, advice, education and information to this specific client group – who mainly have significant communication difficulties due to their illness e.g. thought disorder, hallucinations, delusional ideas, paranoia, and cognitive impairment etc.
- On-going organisation and chairing of all CPA meetings for clients whose care one co-ordinates and the subsequent production and dissemination to others of all relevant documentation generated as a result of the meeting. This is in the absence of family support / appointed care coordinator and whilst under crisis resolution and home treatment team care
- Recognising and respecting a client’s need for independent living as dictated by their abilities, and ensuing all care planned reflects this need.
- Recognising and respecting clients need for gender specific workers and ensuring all care planned reflects this need
- Occasionally dealing with medical emergencies e.g. administering basic life support and/or first aid and calling of emergency services e.g. for those who may have attempted suicide.
- Commissioning of and subsequent support of carers’ services as needed e.g. referral to Making Space family support worker and carers’ breaks.
- Advocating on behalf of the client and/or carer, recognising when more formal advocacy is required and referring the client to the appropriate advocacy service.
- Ensuring transitional protocols are followed i.e. from child to adult services and from elder services and that clients and carers are fully involved and supported during this process.
- Ensure on-going monitoring of a client’s physical health and encouragement of the client to regularly attend planned doctor appointments including clinics others as per NICE guidance.
- Giving basic advice on sexual health issues and contraception to clients on one’s caseload and for the unsupervised referral of clients to specialist services as required.
- Assisting a client to obtain employment and/or a client or carer to examine their personal finances, claim state or work benefits as appropriate and budget money. Provision of support and advice to education facilities in relation to clients with mental health problems as required.
- Providing information and seeking advice for clients and carers and other professionals about medication.
- Providing advice, guidance and support to junior members of medical staff.
- Frequently giving information and specialist advice on caring for clients with mental health problems to others both inside and outside of mental health services e.g. GP’s, Midwives, Health Visitors and Social Services.
- Frequent provision of evidence based information and education to a client and/or carer about mental health problems, issues and care available.
- Forming working partnerships that facilitate the joint assessment, planning, implementation and evaluation of care for those with multiple problems e.g. mental health problems and drug misuse, learning disabilities, physical problems, forensic history and child care issues.
- Forming working partnerships that facilitates the joint assessment, planning, implementation and evaluation of care for those clients not in the community e.g. in hospital or in residential, rehab or respite care.
- Having a wide knowledge of all available services and the frequent unsupervised referral of a client and/ or carer to all other appropriate services as deemed essential to their care and provision of support to attend all subsequent appointments e.g. social services resource centres.
- Assessment of when an interpreter is required and the organisation of an interpreter when needed.
- Liaison with the police, courts (which may require the provision of court reports) criminal justice liaison services, domestic violence services and probation services and coordinating care of clients with these services as required.
- The on-going monitoring of the welfare of any children (as appropriate or possible) involved with a client on one’s caseload and ensuring referral (which may be unsupervised) is made to child services as needed if the child is deemed at risk.
- Having a working knowledge of the mental health act and for adherence to this act in delivery of care to a client e.g. consent to treatment, section 17 leave, section 117 aftercare arrangements and section 25 requirements.
- Assisting the Approved Social Worker in Mental Health Act assessments of clients as required.
- Providing regular contact support and advice to private residential and domiciliary care agencies in the care of clients on ones caseload.
- Frequently completing seven-day (48 hour Network Target) follow-up assessment of clients who have recently been discharged from in-patient care (involves assessment of risk, implementing additional care as needed and completing relevant documentation)
- Liaison with benefits agencies with regards to a clients’ benefit claim and provision of reports as requested and consented to by the client.
- Participation in local and National health promotion activities with clients on ones caseload as appropriate e.g. relapse prevention, healthy eating, weight management, exercise on prescription, stroll to your goal etc.
- On-going need to have an understanding of how a client’s gender, ethnicity or religious beliefs and culture and /or language barriers may affect their presentation and having the ability to adapt nursing care accordingly and refer, unsupervised, to specialist services as required.
- On-going need to identify clients on ones caseload that present a high risk to the public and occasionally making a referral for and participating in a MAPPA meeting and contributing to subsequent public protection plans as agreed in the meeting.
- Providing cover as needed for other crisis practitioners in their absence.
- Ensuring support with activities of daily living to clients as needed in order to enhance their capacities e.g. help with shopping, bathing cleaning etc.
- Ensuring one has specialist knowledge of mental disorder and can deliver a variety of evidence based, therapeutic interventions in relation to this specific client group e.g. psychosocial intervention (including elements cognitive behavioural interventions within this approach) Solution Focused Brief Therapy etc.
- Frequent, assessment, problem-solving work, solution focused brief therapy and crisis intervention in clients with mild to moderate problems.

Role :
- Delivers a comprehensive and effective clinical nutrition and dietetic service in the partners acute hospital services including Gastrointestinal Surgical Service and Young Adult Cancer Unit.
- Provides dietetic assessment, diagnosis, treatment, monitoring and management in the clinical setting and in one or more sub-specialty areas.
- Acts as a source of expertise in nutrition support and dietetics to patients, medical staff, health care professionals, relatives, carers and all relevant agencies.
- Provides expertise to Nutrition Teams and works in other multidisciplinary teams.
- Delivers education and training at all levels.
- Participates in uni- and multi-disciplinary evaluation, audit and research.
- Senior Dieticians will normally work within preferred hospital sites however some outreach clinics and home visits, undertaken independently and/or with other healthcare professionals is expected as can be required by client.
- Very occasionally required to undertake work out of normal hours e.g. to give a lecture or to take part in an outreach clinic.
